On December 23, 2022, our office experienced an extensive flood due to a burst sprinkler pipe and we will not reopen the office at this site due to lengthy delays in repairs. Please see important patient care notices and contact information below and in the links provided. 

  • All patients who are scheduled for upcoming appointments will be contacted by our staff.
  • All recent postoperative patients who need to be seen will be called with additional information. 
  • Medication refill requests should be made via your pharmacy, as usual. The medical assistant will contact you with further information upon receipt. 
  • We are unable to schedule any new patients

If you have an urgent medical problem only, please contact the office by phone and follow the correct prompt to leave a message on the appropriate extension to expedite a return call.

To request that your medical records be sent to another provider, please complete the appropriate form from Dr. Bryan or from Dr. Mettman.

The release of records will not be initiated until we receive your executed record release form. We must receive an executed request for all record transfers, including those to Dr. Mettman, before any action is initiated. Please see Dr. Mettman's 30 day notice letter for additional information. Please do not leave voicemail messages with the staff to request records as we are unable to honor verbal requests.
